Department of Health and Human Services (HHS) and non-HHS organizations are eligible for requesting performances by the USPHS Commissioned Corps Music Ensemble.

Instructions:

2. Complete the PERFORMANCE REQUEST FORM (click bolded text).
3. Email the Performance Request Form to request"at"phsensemble.org.
